After your initial appointment, we offer these three Follow-up packages:
Three Individual Clinic Visits - $800

Six Individual Clinic Visits - $1,400

TELEHEALTH VISIT
Step 1. Call the clinic at (615) 624-4489 or email us at info@BoroHH.com to schedule a telehealth appointment.
Step 2. Download and Print two required forms, the NP Intake form here and PMA form here.
Step 3. Once the Biological Blueprintâ„¢ packet arrives, follow all instructions provided. Be sure to send the New Patient Intake form and PMA form with biological samples.
Step 4. In the self-addressed and pre-stamped envelope, mail complete forms and biological samples to:
